MIDDLETOWN CHAMBER OF COMMERCE SECOND ANNUAL GOLF SCRAMBLE ENTRY FORM

CHARITY:        Kentucky Wounded Heroes, Inc. a 501(c)(3) non-profit entity that exists to serve Kentucky’s current or former members of the United States Armed Forces, sworn law enforcement, sworn firefighters, and sworn or affirmed EMS personnel, all of whom have been injured during combat operations or line-of-duty service.  They provide opportunities to hunt, fish, travel and to connect to nature’s healing properties. 

ENTRY FEE:     $100.00 (includes: green fees, cart & catered lunch & reception with hors d’oeuvres & prizes afterwards Payment Due:  April 30, 2018 (no refunds)

Send Check to the Middletown Chamber office or call with cc #.  Proper golf attire required (NO denim (jeans) and a collared shirt (or golf mock) is required.

Format:  Tournament will be played in a best ball scramble with 4-person teams. Shot gun start.  If you do not enter a team list, you will be placed with others. 

Cancellation Policy:

If nine holes of the event are completed by all players & inclement weather persists, the event will be considered complete, with no refunds.  If the club pro deems the course is unplayable with less than nine holes played, the scramble is cancelled with no refunds.
